Hello!
I am Dr. Tak Seung-wan, the head director of Dr. Tak Plastic Surgery Clinic, specializing primarily in lip surgery in Gangnam, Seoul.
Since I mainly focus on lip surgery, I often encounter patients in my consultation room who are concerned about their downturned lip corners.
Recently, they often hear things like,ย "Why are you angry?" even when they have a neutral expression. To address these concerns, many people first try non-surgical methods like thread lifts.

However, based on my actual consultations, most people who undergo thread lifts experience minimal effects and eventually choose surgery. Today, based on my experience of performing procedures/surgeries on over 1,500 patients for more thanย 10 years, I would like to honestly talk about the differences between 'lip corner thread lift' and 'lip corner plastic surgery'.
If you take the time to read this post, it will be of practical help in resolving your concerns.
Lip Corner Thread Lift vs. Surgery: Explanation from a Board-Certified Plastic Surgeon
1. Overall Explanation of Lip Corner Thread Lift and Reasons for Minimal Effectiveness
Lip corner thread lift is, a procedure that lifts the skin and soft tissue around the corners of the lips using special threads.ย

It is performed under local anesthesia within 30 minutes, and patients tend to prefer it because the threads are inserted with needles without significant incisions. However, to be honest, the effects of a lip corner thread lift are often less significant than expected.
Why is that?
Firstly, the fundamental cause of downturned lip corners is often not just simple skin sagging. When I directly examine and diagnose patients' lips, the lip corners are formed at the intersection of several muscles (especially the depressor anguli oris and the levator anguli oris muscles), and in most cases, this muscle structure itself is congenitally positioned lower.

However, thread lifts do not affect this muscle structure and only pull the surface tissue. Therefore, it is difficult to fundamentally solve the problem in such congenital cases.
Secondly,ย
the effects of thread lifts are too temporary. In my experience, they usually return to their original state within 3 to 6 months. The area around the lips moves thousands of times a day, making it a difficult environment for the threads to maintain their hold. Therefore, there is the inconvenience of having to repeat the same procedure frequently.
Thirdly,ย
the reason why many women's lip corners appear downturned is often due to theย 'triangle lip' shape, where both ends of the upper lip curl inwards. And this problem cannot be solved at all with a thread lift. Performing a thread lift does not cause the curled lips to flatten upwards.

So, in such cases, you should consider a different procedure rather than a thread lift.ย
Lip Corner Thread Lift vs. Surgery: Explanation from a Board-Certified Plastic Surgeon
2. Reasons for Recommending Lip Corner Plastic Surgery Over Lip Corner Thread Lift
Now, I tend to recommend lip corner plastic surgery whenever possible. This is because lip corner plastic surgery is definitely effective. Lip corner surgery does not simply pull the skin, but also approaches and corrects the muscle structure around the lips, allowing for the fundamental resolution of the problem.

When undergoing lip corner surgery, the lip corner area is incised in aย < > shape, the skin is appropriately excised, and the depressor anguli oris muscle within is either released or repositioned.ย
As a result, the structural position of the lip corners themselves is changed, allowing for permanent effects.

Ah! For your reference, the most important thing I consider during surgery is a customized design that suits the patient's face shape and facial features.
Simply lifting the lip corners a lot is not necessarily beautiful; rather, it is about finding the most natural lip corner position by considering each patient's facial balance and proportions.
Only through such a thorough process can you see definite results, so I recommend going to a place with a meticulous consultation process.
Lastly,ย
for those whose lip corners appear downturned due to a triangle lip shape, it is better to undergo a surgery calledย 'lateral philtrum shortening' rather than simply getting lip corner surgery.
After undergoing this surgery, the curled lips can be flattened, resulting in the effect of the lip corners being lifted. As such, there are different suitable surgical or procedural methods depending on each type, so please keep this in mind when deciding on lip corner-related surgery/procedures.
So far, I have explained the differences between lip corner thread lift and plastic surgery.
Based on my clinical experience, if you want to resolve the fundamental cause of downturned lip corners and desire lasting effects, I believe that surgery is a more suitable choice. Especially if you have tried thread lifts multiple times without success, it would be good to consider plastic surgery, which can provide permanent correction with a single procedure.
Of course, whichever method you choose, I recommend that you definitely consult with a specialist with extensive experience in the field before making a decision.
Even if you don't come to our clinic, it is most important to receive an accurate diagnosis and make a decision at a clinic specializing in lip surgery, so please keep this in mind.
