Hello!
I am Tak Seung-wan, the representative director of Dr. Tak Plastic Surgery, primarily specializing in philtrum reduction surgery in Gangnam, Seoul.
When the philtrum (the vertical groove between the base of the nose and the upper lip) is long...
The face can appear longer, look older, and especially for women, it can give a masculine impression, which is often undesirable. That's why many people look into philtrum reduction surgery, and at this time, the biggest concern is none other than 'scars'.
In fact, during consultations, I very frequently receive questions like, "Is there a way to reduce the philtrum without leaving a scar?"
To tell you the conclusion first... It is practically difficult for a completely scar-less philtrum reduction method to exist.ย
(It's somewhat like trying to find a way not to gain weight while eating late-night snacks...)
While there are surgical methods known as scar-less philtrum reduction, they have various limitations.
So, for those who are concerned about this, today I'd like to tell you about the limitations of the method known asย 'scar-less philtrum reduction'ย and 'realistic methods to minimize scars after philtrum reduction surgery'.
If you are looking into scar-less philtrum reduction, I hope you will read this article thoroughly.
1. Surgical method of scar-less philtrum reduction, and its limitations.
First, let's look at the method known as 'scar-less philtrum reduction'.
The method known as scar-less philtrum reduction is a type of procedure that involvesย cutting away the flesh under the nose,ย and it is sometimes said that the surgery leavesย no visibleย scars on the surface.
This method involves removing theย 'protruding flesh at the bottom of the nose' located inside the nostrils.

This has the advantage that the scar remains inside the nostril, making it not visible from the outside. However, this method has several clear limitations.
First,ย the incision area is very small, so the philtrum reduction effect is limited. Compared to general philtrum reduction surgery, which involves incising and lifting the philtrum, the range that can be reduced is much smaller, so the post-operative effect may be negligible.
Second, there is a risk of nostril shape deformation. The flesh under the nose plays a quite important role in determining the shape of the nostrils, so removing it can make the nostril shape unnatural.
Third, it cannot be applied to everyone. This method is only possible for people with sufficiently developed flesh under the nose, so only about 40-50% of people can attempt this method.

Furthermore, because this method is relatively newer compared to general philtrum reduction, sufficient academic verification has not yet been thoroughly conducted, and clinical experience is relatively insufficient. And as it hasn't been around for long, data on long-term results and complications are also insufficient.
And most importantly, there is a high probability that even if you undergo surgery with this method, you may not achieve the desired philtrum reduction effect. (Because the incision area is small.) So, you might end up needing to undergo general philtrum reduction surgery again, which could result in a loss in terms of time and cost.
Therefore, I would recommend that you receive philtrum reduction using traditional methods if possible, and then manage the scars well afterward to make them almost invisible.

2. Methods to reduce philtrum reduction scars.
So, what are the realistic methods to minimize scars after general philtrum reduction surgery?
General philtrum reduction involves incising the skin along the line under the nose, removing the necessary amount of philtrum skin, and then suturing.
Therefore, a scar remains under the nose in this process...ย but it can be minimized by the following methods.
First, using a special suturing method. By using a suturing method called 'Vertical mattress suture', the incision site is sutured slightly elevated like a mountain,

which allows the scar to maintain a smooth surface even if it widens over time. This method requires more than 4 times the number of stitches compared to general suturing, but it is highly effective in minimizing scars.
Second, firmly fixating the skin to the central ligament under the nose during suturing.

This disperses the tension applied to the suture site, preventing the scar from widening.
Third, consistent scar management after surgery. Through scar laser treatment, scar injections, Botox injections, and appropriate massage, scars can be significantly faded within about 3 to 6 months.

For reference, this scar management usually begins aboutย 1 to 2 weeks after surgery, once the stitches are removed. Approximatelyย 6 to 8 weeks after surgery, laser treatment to reduce scar redness can begin.
With proper management, in most cases, the redness disappears after 2 to 3 months post-surgery, and after about 6 months, the scar becomes faint enough to be sufficiently covered with light makeup. Of course, the degree of remaining scar may vary individually, but in most cases, it can be sufficiently improved to the point where it can be covered with light makeup.

Today, I talked about scar-less philtrum reduction surgery.
In conclusion, a completely scar-less philtrum reduction is realistically difficult. There are clear limitations.
However, general philtrum reduction surgery can now minimize scars through special suturing methods and consistent scar management, so it would be good to decide after thoroughly researching clinics.
