As I consult with patients in my examination room every day,

I often hear many people say,ย "My philtrum is long, and it bothers me..."

If the philtrum is long, the face can look elongated, or it can make you look older than your actual age.


So today, for those with such concerns, I'd like to share two important pieces of information you should definitely know when considering philtrum lifting (philtrum reduction).


Especially, 'what young individuals and middle-aged individuals should consider respectively,'

I will speak honestly based on the expertise I've gained from treating over 1,500 patients to date.


Let's read this together with a moment of focus.



1. Information for Young People Who Desire Philtrum Lifting.


Most young people in their 20s and 30s often have a congenitally long philtrum.


For these individuals, 'internal philtrum reduction' surgery is most effective.

However, one truly important thing is that,ย shortening the philtrum unconditionally does not necessarily make you look prettier.


Among the people who come to my clinic, many say, "Doctor, please make my philtrum as short as possible!"


In fact, the overall proportion of all facial features is important.

A harmonious vertical ratio of the philtrum, lips, and chin is essential for an attractive face.ย 

Therefore, when a patient visits, I always consider the overall balance of their face and advise,ย "This much would suit you best."

Sometimes, even if a patient desires philtrum reduction, if I determine it doesn't suit their face shape, I honestly tell them and recommend other procedures instead.

This is because surgery might not make a significant difference, or could even result in an unnatural look.


Also, the biggest concern for young people are scars.

Philtrum reduction inevitably leaves a scar along the line under the nose.


However, through my specialized suturing technique and post-surgical care (laser, scar injections, Botox, etc.),

the scar significantly fades after 3-6 months, making it possible to conceal it with light makeup.

For your information, please remember that it is particularly difficult for young people to see significant effects with non-surgical methods (such as fillers, Botox, or Shurink/Hifu)

For congenitally long philtrums in young individuals, surgical methods are the most certain.



2. Information for Middle-Aged Individuals Who Desire Philtrum Lifting.


The case for middle-aged individuals in their 40s and 50s is a bit different.

In most cases, the philtrum lengthens as part of the aging process.


As we age, the collagen that maintains skin elasticity in our face decreases, and the ligaments supporting the face graduallyย sag downwards. This process causes the philtrum to lengthen.

For such middle-aged individuals, there are two options.


The first is a non-surgical method. High-frequency lifting procedures like Ultherapy or Shurink/Hifu Laser promote collagen production within the skin and lift sagging support ligaments, which has some effect in reducing philtrum length.


If the philtrum has only slightly lengthened in the early stages due to aging, such non-surgical methods can provide some improvement.


However, if the philtrum has become significantly long, internal philtrum reduction surgery is also effective for these individuals.


For middle-aged individuals, there's one more thing to consider. Among middle-aged people, not only does the philtrum lengthen, but sometimes the outer edges of the upper lip also roll inwards.

In such cases, simply reducing the philtrum might not yield satisfactory results.


Therefore, I often recommend performing both internal philtrum reduction and lateral philtrum reduction simultaneously.

Lateral philtrum reduction is a surgery that inverts and enlarges the rolled-in outer part of the upper lip, which helps to make the upper lip appear fuller. Many middle-aged individuals who receive both of these surgeries together often say they hear comments like, "Your face looks much younger!"



So far, I've shared essential age-specific information you should know regarding philtrum lifting (philtrum reduction).


To summarize, for young individuals, customized surgery considering facial proportions is important, while for middle-aged individuals, a comprehensive approach that considers not only the philtrum but also changes in the lips is necessary.


The philtrum is a small area, but it's located at the center of the face, and even a small change in this area can significantly alter the overall facial impression.

Therefore, it's a decision that should be made carefully.

Before deciding on surgery, I recommend visiting several clinics for thorough consultations and choosing the optimal method that suits your face shape and age.
